The following quick self-assessment questionnaire is based on the formula in this web site. It is intended to provide you with an appraisal of your current status and will allow you to estimate in which areas you can focus to improve your ability. Your decision to work on a personal area of weakness could help to shape your championship potential.

As you work through this assessment try to be as objective as you can in awarding yourself a score for each of the following categories that are taken from the formula discussed above. Try to realize that a true self-assessment could lead you to improve areas in your life that will bring significant improvement in your performance as a student athlete!

Everyone is capable of improvement; however, achieving excellence in the areas discussed above requires awareness of one's current attitudes and effort. The time to begin is NOW! After computing your score, assess your current standing and discuss your observations with your coach, team athletic trainer, professor, or athletic administrator. Develop a personal plan of action.
